​To join us for any session, program, retreat or ceremony, please submit the 4 required items listed below.

If you are  without software for completing the subject .pdf forms online, or if you do not have the means for  downloading, scanning and sending them to us via email, you may want to click through to the website CONVERT PDF TO WORD. There, after you save each of our .pdf forms to your device, you will be able to  convert each .pdf  into Word without charge and complete the form in Word. This allows you to then email each completed form to us as a Word attachment. Our email address is at the top of each form. 

Submit These 4 ITEMS:

  • A  scan or pics of your signed, completed APPLICATION / INFO / WAIVER FORM

  • A real-time, in the moment head & shoulders 'selfie' that you take specifically for this purpose (nothing from your files, please) with eyes exposed -- no glasses, looking directly into the camera lens,  

  • A scan or image of your photo ID (drivers license, passport, etc.), 

  • Your "Family & Friends" charitable contribution, which may be submitted to our Gifting Fund via instructions at  the "Donate" button in the bottom right-hand corner of this page or at the following link:

For anyone in the USA or Canada without a health insurance plan in place, International Volunteer Card at https://www.volunteercard.com/compareplans/ offers a suitable, effective, 60-day plan to anyone taking a trip of 100 miles or more for the purpose of non-profit organization projects. [That's us! Muaisa Hale Pule is an approved 501(c)3.] The plan, suitable for participant involvement, costs only $30 US and covers everything from accidental medical, dental and illness to trip delay and baggage loss or delay. Moreover, IVC has longer term plans as well, if you need one, and they donate a significant portion of their proceeds to humanitarian projects.

The easiest way to submit the required list of items above is as follows:

  • Print out, fill in, and scan or photograph your signed, completed APPLICATION / INFO / WAIVER FORM

  • Email the completed form, along with the requested additional info, to the email address at the top of the form,

  • If you are using an Apple, or if you happen to have Adobe Writer, you may be able to fill in the form(s) online,
