# 1: START BY SUBMITTING THESE 5 ITEMS
 

To join us for any program, ceremony, or session, you will need to first submit the following:

1.  A  scan or pics of your signed, completed Waiver of Liability Claims Form.
2.  A real-time, in the moment head & shoulders 'selfie' that you take specifically for this purpose (nothing from your files, please) with eyes exposed -- no glasses, looking directly into the camera lens,  

3.  A scan or image of your photo ID (drivers license, passport, etc.), 

4.  A scan or pic of your current health insurance card or proof of current health insurance coverage (see instructions below if you do not  currently have insurance coverage)*,
5. Your "Family & Friends" charitable contribution, which may be submitted to our Gifting Fund via instructions at  the "Donate" button in the bottom right-hand corner of this page or at the following link:

* For those without a health insurance plan in place, International Volunteer Card at https://www.volunteercard.com/compareplans/ offers a suitable, effective, 60-day plan to anyone taking a trip of 100 miles or more for the purpose of non-profit organization projects. The plan, suitable for this purpose, costs only $30 US and covers everything from accidental medical, dental and illness to trip delay and baggage loss or delay. Moreover, IVC has longer term plans as well, if you need one, and donates a significant portion of their proceeds to humanitarian projects.

# 2: ADD THE APPLICATION  FORM OR FORMS FOR YOUR CHOICE OF PROGRAMS 

 

Muaisa Hale Pule offers a variety of separate, a la carte programs and ceremonies, most of which require pre-application, based on intention, lifestyle and general well-being. 

 * * * * If you intend to participate in the Shamanic Rebirth or a volunteer program + a plant ceremony, you will need to complete and submit two (2) separate application forms. * * * *

The easiest way to submit the required list of items is as follows:

  • Print out, fill in, and scan or photograph your signed, completed Waiver of Liability Claim Form,

  • Email the completed form, along with the requested additional info, to the email address at the top of the form,

  • If you are using an Apple, or if you happen to have Adobe Writer, you may be able to fill in the form(s) online,

  • Most completed application packages are acknowledged and accepted within 24 hours of receipt.

Anyone wanting to participate in 2 or more programs (ex., 'Hawaiaka' Sacred Plant Ceremony + 10-day Shamanic 'Life Purpose' Rebirth) will to complete and submit two (2) specific application forms.

Complete and submit this form along with the 5 items in Section #1 above. Ceremonies take place every Sunday evening throughout the year, based on a pre-enrollment of 2 - 6 participants. Please submit this application form along with  the 5 items listed above in Section #1.

This is our 10-day, flagship, 'Life Purpose' tutorial. Once you complete this individually-tailored  work and "get it," you will never need to take another seminar or workshop. You can, of course,  go wherever you choose for the entertainment value-- but you will know once and for all how to access your own Inner Wisdom without needing to seek or require external validation. In conjunction with this application form please also submit the 5 items listed above in Section #1.
