Muaisa Hale Pule offers a variety of separate, a la carte programs and ceremonies, most of which require pre-application, based on intention, lifestyle and general well-being.

 * * * * If you intend to participate in the Shamanic Rebirth or a volunteer program + a plant ceremony, you will need to complete and submit two (2) separate application forms. * * * *

The easiest way to submit an application and/or waiver form is to print out, fill in, and scan or photograph your signed, completed pages. Then send them via email to the address at the top of the first page on the form. Depending upon your computer (Apple) and programs (Adobe Writer), you may be able to fill-in the forms online. Most completed application packages are acknowledged and accepted within 24 hours following submission. 

• • •  PART I APPLICATION FOR "HAWAIAKA" SACRED PLANT CEREMONY

Complete and submit this form along with a real-time head & shoulders selfie, a scan of  your photo ID, and  + + + PART II SIGNED WAIVER / CONSENT FORM.  A minimum of 2 participants is required for each ceremony. Maximum number of spots available per ceremony = 6.  

• • •  APPLICATION FOR SHAMANIC REBIRTH & HEALER CERTIFICATION

This is our 10-day, flagship, 'Life Purpose' tutorial. Once you complete this individually-tailored  work and "get it," you will never need to take another seminar or workshop. You can, of course,  go wherever you choose and study with whomever you'd like -- for the entertainment value-- but you will know once and for all how to access your own Inner Wisdom without needing to seek or require external validation. Please submit this application form along with  a real-time selfie and a scan of your photo ID.
